{"id":2576,"date":"2023-10-05T11:52:12","date_gmt":"2023-10-05T08:52:12","guid":{"rendered":"https:\/\/consgerusalemme.esteri.it\/?page_id=2576"},"modified":"2025-12-15T11:35:38","modified_gmt":"2025-12-15T09:35:38","slug":"change-of-name-and-or-surname","status":"publish","type":"page","link":"https:\/\/consgerusalemme.esteri.it\/en\/servizi-consolari-e-visti\/servizi-per-il-cittadino-italiano\/change-of-name-and-or-surname\/","title":{"rendered":"Change of name and\/or surname"},"content":{"rendered":"<header class=\"entry-header\">\n<h1 class=\"entry-title h3\"><span style=\"font-size: 16px;\">You can request recognition of the change of name and\/or surname by following these simple steps:<\/span><\/h1>\n<\/header>\n<div class=\"entry-content\">\n<p><strong>STEP 1 \u2013 Preparation of documents<\/strong><\/p>\n<p>Collect the following documents:<\/p>\n<ul>\n<li>Motivated request for recognition of the change or modification of one\u2019s name and\/or surname addressed to the competent Prefect in Italy;<\/li>\n<li><a href=\"https:\/\/ambtelaviv.esteri.it\/wp-content\/uploads\/2023\/08\/2023_dichiarazione_sostitutiva_di_certificato-o-atto_di_notorieta.pdf\">Declaration of notary deed<\/a>\u00a0to confirm the non-existence of pending charges in Italy and the compliance of the procedure with local laws;<\/li>\n<li>Declaration of consent of any interested parties such as parents, spouses and adult children, accompanied by a photocopy of their identity document (only in the case of change or modification of the surname);<\/li>\n<li>Copy of your Italian passport;<\/li>\n<li>Copy of your Israeli passport or \u201cteudat zeut\u201d;<\/li>\n<li>Original certificate of change of name and\/or surname (in Hebrew \u201cTeuda Ameida al Shenui Shem\u201d) with Apostille\u00a0from the Israeli Ministry of Foreign Affairs (for more information on how to obtain an Apostille\u00a0<a href=\"https:\/\/www.mfa.gov.il\/mfa\/consularservices\/pages\/publicdocumentseng.aspx\">click here<\/a>);<\/li>\n<li>Translation into Italian of the certificate of change of name and\/or surname.<\/li>\n<\/ul>\n<p><strong>The application is processed upon payment by bank transfer of the \u201cImposta di bollo\u201d to be made using the following bank details and with any associated fees borne by the payer:<\/strong><\/p>\n<blockquote><p>BENEFICIARY: Ministero dell\u2019Economia e delle Finanze<\/p>\n<p>IBAN: IT79Z0100003245BE00000000QK<\/p>\n<p>BIC: BITAITRRENT<\/p>\n<p>REASON FOR TRANSFER: \u201cPLEASE INSERT HERE YOUR FISCAL CODE\u201d \u2013 Bollo su istanza Prefettura \u2013 mese e anno di riferimento (INSERT HERE THE MONTH AND THE YEAR IN THE FORMAT MM\/YYYY)<\/p>\n<p>AMOUNT: 16 euros<\/p><\/blockquote>\n<p>The payment has to be made before bringing the documents to the Consulate. The Swift Message of the payment will be presented together with the other documents.<\/p>\n<p><strong>STEP 2 \u2013 Verification of documents and sending to Italy<\/strong><\/p>\n<p>Verify personally with our Consulate staff that the above documents are complete by requesting an appointment at consol.gerusalemme@esteri.it. The day of the appointment you will pay a fee for the consular stamp (see \u201cConsular services\u201d page). After that, the Consulate will send the application to the competent Prefecture in Italy.<\/p>\n<p><strong>STEP 3 \u2013 Appointment for the posting of the \u201cRequest for change of name or surname\u201d<\/strong><\/p>\n<p>Once the authorization to post the change of name and\/or surname has been received from the competent Prefecture, the Consulate will contact you to make an appointment to fill in and sign a \u201cRequest for change of name or surname\u201d. The Consulate will then proceed to post the Request to the Consular Register to allow for any objections. The posting will last for thirty days.<\/p>\n<p><strong>STEP 4 \u2013 Issue of the \u201cAuthorization Decree\u201d<\/strong><\/p>\n<p>At the end of the 30 days, you will receive from the Consulate the original of the \u201cDecree authorizing the change or modification of the name or surname\u201d, stamped with a new consular stamp (see \u201cConsular services\u201d page).
